Physical Therapy
The following are rates for my cash-pay patients. See below for information on insurance coverage.
- Yearly Preventative “Movement” Physical 90 minutes $250
- Physical Therapy Evaluation 60 minutes $200
- Includes thorough analysis of posture, alignment, gait, soft tissue quality, strength and flexibility.
- You will walk away with a treatment plan and initial home program
- Physical Therapy Follow-up visit 55 minutes $150
Elsa Gould Physical Therapy, PLLC accepts the following insurance plans and will directly bill on your behalf:
- Blue Cross Blue Shield PPO plans
- Medicare (with the exception of Medicare HMO plans)
**I am out-of-network (OON) at this time for all other insurance plans. If your plan includes OON benefits, I am happy to provide a Superbill to help you seek reimbursement from your insurance company.
Pilates
Pricing:
**3 trial sessions are available to new clients with “a la carte” payments. After 3 sessions, either a 5 or 10-pack package is required in order to reserve your time slot. Your package will be pro-rated to bring your first 3 sessions down to the package rate you select.
Private sessions: $100 a la carte, 5-pack $475 ($95/session), 10-pack $900 ($90/session)
Duet sessions: $65 a la carte per participant, 5-pack $300 ($60/session), 10-pack $550 ($55/session)
Group classes: $50 a la carte, 5-pack $210 ($42/session), 10-pack $450 ($45/session)
